Responsibilities:
Assists residents with emotional adjustment to facility.
As directed, interviews new residents and records social history and assessments.
Completes the social services portion of the MDS.
Schedules and Attends and participates in interdisciplinary care plan meetings.
Writes summaries and progress notes as directed.
Assists in conducting a pre-discharge evaluation Assists with discharge planning.
Follows up after discharge.
Writes discharge summaries as assigned.
Develops a relationship with each resident
Qualifications:
One to Three years of professional experience and/or training; or equivalent combination of education and experience.
